Septage Hauling Permit Application

In accordance with MGL c. 111, section 31 B and 310 CMR 15.502 (Title 5) the applicant makes application to the Oakham Board of Health for permission to remove and transport septage and the content of privies in cesspools as set forth below.

I understand the fee for a Septage Hauler Permit is $50.00 and this permit expires upon approval on December 31st of the calendar year application is made. This application will not be processed without receipt of the fee and required documentation (if applicable).
I certify that the information I have provided above is true and accurate. I recognize that it is a violation of this permit to dispose of septage anywhere other than the identified disposal location(s) approved by the board in writing as an amendment to this permit. Attest by entering your name.
