Oakham Board of Health Permits & Applications
Permits/Applications/Plans should be mailed to the Town Hall or placed in the outside box in front of the Town Hall (attention BOH). The BOH does not have staff or office hours, BOH members do meet in person during public meetings which are held monthly. Feel free to notify the boh@oakham-ma.gov email if you have dropped off or mailed an application and fee.
Well Permit Application
- Well Construction Permit (can be completed online - with payment mailed).
Title V Permit Applications:
- Soil Evaluation and Percolation Test Application Form
- Application for Disposal System Construction Permit Form 1A
- Disposal System Construction Permit Form 2A
- Certificate of Compliance Form 3
- Septic Installer Permit
- Title V Inspection Form
Commercial Hauler Permits:
- Septage Hauler Permit (can be completed online - with payment mailed).
- Refuse Hauler Permit
